The Challenge

For a CMS-certified ACO, quality benchmarks aren’t a scorecard: they’re the revenue model. Payer contracts and shared-savings arrangements hinge on hitting population-level targets (cancer screenings, hypertension control, diabetes management) on schedule, every year. Falling short on any one measure affects performance across the entire network.

Care gaps impact far more than quality incentives - they influence revenue, retention, risk adjustment, and attribution.

Integra’s problem wasn’t knowing which patients were due. It was reaching all of them, on schedule, without exhausting its care coordination resources. Across a network of more than 200 primary care providers, manual outreach was inconsistent, hard to prioritize, and slow to scale. At the same time, leadership needed to protect referral integrity: keeping services like screening mammograms at Care New England’s own facilities rather than losing them to outside providers.

Care New England and Integra brought in HealthHelper to run data-driven patient outreach and appointment scheduling, integrated directly with their Epic EHR. Implementation was complete in under two months, letting the ACO begin closing quality gaps well ahead of year-end reporting deadlines.
